Anaphylactoid or Pseudoallergic

Anaphylactoid or pseudoallergic reactions to food result from foods that mimic the effects of mast cell degranulation but do not involve IgE antibodies. Strawberries and shellfish may cause this type of ARF. Certain food ingredients, including additives such as salicylates, benzoates, and tartrazine, induce pseudoallergic reactions. As with true food allergy, patients exhibiting such reactions should be instructed to avoid the offending food substance if identifiable. Pharmacological reactions to food or food additives represent a relatively common type of ARF, although most of these reactions cause symptoms outside of the GI tract. Histamine found in certain cheeses or in scrombroid fish, such as tuna, can cause headaches and diffuse erythema of the skin. Certain individuals develop migraine headaches to various foodstuffs, including those rich in amines. An increase in roughage can be achieved not just by increasing the percentage of vegetable foodstuffs ingested, but also by choosing products with more roughage. As fruits and vegetables largely consist of cellulose, such a substitution is limited, because the roughage in fruits and vegetables, with the exception of pulses, is only around 1-3 7,8 . Berries and dried fruits have the highest roughage content. The vegetables with the highest amount of roughage are green peas, leeks, cabbages, and pulses however, they are also more indigestible and can lead to flatulence. One should eat four to five portions of vegetables, uncooked vegetables, salads, fruit, and or fruit juices every day. Foodstuffs that consolidate the stools, such as bananas, blueberries, boiled carrots, rice, low-fat curd cheese, hard-boiled eggs, cocoa and black tea, should initially be avoided. If the symptoms improve it may be possible to reintroduce them.

Prevention Dietary

Being able to eat and drink only clean food and water clearly will protect the traveler from diarrheal illness. Unfortunately this is often impossible to do, because food preparation is being done by the staff of the hotels, resorts, and cruises. A few simple suggestions may help in avoiding the riskiest foods. Avoid fresh vegetable salads, unless they have been disinfected. (This is usually not done in restaurants, but can be done easily in a home kitchen.) Avoid eating fruits that cannot be peeled, such as strawberries or grapes fruits that can be peeled, such as bananas or oranges are safe no matter where they come from. Avoid eating poorly cooked meat or fish. Avoid eating from food vendors on the street, unless the food is piping hot. Remember that coffee and tea are always safe, because the water has been heated during preparation (Table 50-2).

Theobromine is found in cacao plants but not in the final products based on cacao seeds, such as cacao drinks or chocolate. Theobromine is removed during fermentation and processing. Processing is very important for the production of the final alkaloid product consumed. In the case of coffee, a high-quality product is possible only from the ripened berries600'601.
