Merely mouthing platitudes about the importance of research is not enough. We can talk about research all we want, but if the culture of the department and school says that clinical service is what it is all about, research will tend to suffer. We need to make sure that we offer role models of successful physician-researchers. When we teach learners at the point of care, we need to encourage the formulation of good investigative questions. Journal clubs are an important opportunity for medical schools and residency programs, because they develop habits of reading and critically appraising the medical literature. Residents should also be monitored to ensure that they learn to assess and address knowledge deficits as well as learn healthy habits in responding to errors. As as example of a cluster randomized trial, we describe the Child and Adolescent Trial for Cardiovascular Health (CATCH). Details concerning the design of this trial are reported in Zucker et al. (1995), and the main trial results are reported in Luepker et al. (1996). The CATCH study investigated a school-based educational and environmental intervention aimed at promoting heart-healthy habits in elementary school children.
The diabetes preventive care state (top half of Fig. 3) focuses on providing regular screening for early detection of symptoms and complications initial diagnosis of type II diabetes and preventive services to help patients develop and maintain healthy lifestyle habits, and minimize the risk for complications.
Preventive care aims to improve delivery of appropriate clinical preventive services by helping patients to maintain and or develop healthy lifestyle habits and eliminate high-risk behaviors. In the context of managing type II diabetes, there are several health factors that require close observation, e.g., blood pressure, lipid management, antiplatelet agents, smoking cessation, coronary heart disease (CHD) and immunization. These factors are explained in more detail in the following sections.
By contrast, destructive emulation occurs when learners adopt habits that undermine their excellence. Consider a disgruntled and frankly cynical faculty member, whose residents tend to develop such habits as criticizing colleagues behind their backs, thereby corroding collegiality and mutual respect within the department. One goal of all medical educators should be to cultivate opportunities for constructive emulation and reduce opportunities for destructive emulation. We need to consider not only the content of the formal curriculum, but that of the informal and even hidden curriculum, as well. With whom are learners working, and to what effect One way of enhancing our educational effectiveness as role models is to strengthen our understanding of this vital but often overlooked aspect of education.
