Balloon Tamponade

Balloon tamponade is a highly effective method of controlling active variceal bleeding. In experienced hands, it is equivalent to endoscopic treatment for the immediate control of bleeding, but efficacy extends only to the period of application. In inexperienced hands, the morbidity and mortality of balloon tamponade may be unacceptably high. Balloon tamponade is best reserved for the management of life threatening bleeding when a risk of exsanguination exists and endoscopic intervention is unlikely to be feasible, or when a combination of endoscopic and pharmacologic treatment has failed to control bleeding. Balloon tamponade should be followed by more definitive treatment within 24 hours. Depending on the situation, the definitive treatment may include endoscopic ligation or sclerotherapy (only in those who had no effective endoscopic treatment prior to balloon tamponade), surgery, and TIPS.
