Oral fluoropyrimidines offer a more convenient alternative to continuous infusion 5-FU. 5-FU has been unsuitable for oral delivery due to its poor bioavailability and rapid catabolic clearance by dihydropyrimidine dehydro-genase. Capecitabine is an oral 5-FU prodrug that is well absorbed by the GI tract and undergoes a three-step enzymatic conversion to active 5-FU. Two randomized trials of first line twice-daily oral capecitabine with bolus 5-FU/LV in patients with advanced CRC have reported equivalent survival efficacy with a more favorable toxicity profile for capecitabine (Hoff et al, 2001; Van Cutsem et al, 2000). Capecitabine thus represents a reasonable option for patients who would be candidates for treatment with 5-FU/LV alone.
