Clinical Phenotypes

When CD is limited to the SB, it usually involves terminal ileum. The other segments of the SB are affected in approximately 20% of the cases. Given the frequent terminal ileal involvement, the classical symptoms of SB CD are right lower-quadrant abdominal pain and diarrhea. Other common symptoms include low grade fever, decreased appetite, fatigue, and weight loss. Patients with more diffuse inflammatory pattern of the SB, including fistulizing disease, have more prominent components of malabsorption, such as diarrhea and weight loss. in contrast, patients with fribrostenotic disease usually present with symptoms of partial SB obstruction, including abdominal pain, bloating, nausea, and vomiting.
