Esophageal dilatation is a safe and effective therapy for the management for esophageal strictures with an overall complication rate of < 2%. It is important to properly assess the characteristics of the stricture in preparation for the dilatation procedure. Blindly passed dilators are suitable only for simple, short and symmetrical strictures with little risk for complications. Polyvinyl and hydrostatic balloon dilators are used for complicated, angulated, and long strictures, as well as refractory strictures. A guidewire and/or fluoroscopy aid in improving the efficacy and safety of the procedure. Peptic stricture recurrence can be markedly reduced by effectively inhibiting acid reflux with the long term use of high dose PPI therapy.
