Intestinal and colonic strictures require thorough evaluation using endoscopic and other imaging modalities to define benign versus malignant disease. Good surgical risk patients with potentially resectable malignancy should undergo resective surgery, although preoperative decompression with nasogastric suction, stricture dilatation, a colon decompression tube, or preoperative SEMS insertion may decrease postoperative complications or need for a two-stage operation. In unresectable patients with malignant strictures, SEMS offer an alternative to current surgical bypass, decompressive gastrostomy, or comfort care measures. With technologic refinements to include smaller delivery devices and increased use of TTS technology as well as refinements in the prostheses and the accuracy in their delivery, I predict that SEMS will be used with increased frequency, despite the lack of controlled clinical trials.
