Corticosteroids

Corticosteroids in childhood CD are very effective in controlling the inflammatory state and inducing clinical remission. Children and adolescents, however, are adversely affected by the growth-suppressing and cosmetic side effects of corticosteroids. Avoiding repeated use of corti-costeroids is thus optimal but problematic due to relatively high relapse rates. our tendency has been to use more aggressive immunomodulator maintenance therapy earlier in the course of moderate to severe disease in the hopes of reducing long term steroid exposure. Although antibiotics are used in the initial therapy of moderate to severe disease, there is currently little data to support their use.

Tumor necrosis factor-a antibody might also be an alternative, but long-term effects are not yet known. The use of 5-aminosalicylic acid (ASA) drugs to induce and maintain remission has been successful in mild distal CD. However, many of our patients do not respond, or may relapse frequently, and will need corticosteroids.
