Family History of GC

This is one of the most difficult value judgement calls. There are families with the intestinal type of GC, more so in countries with higher GC risks (Caldas et al, 1999). Familial intestinal type of GC has been defined as (1) at least 2 first- or second-degree relatives affected, with 1 diagnosed before the age of 50 years or (2) 3 or more relatives with intestinal type of GC at any age. Concern arises even if the strict definition is not met (eg, when two first-degree relatives have had GC).

There are no studies that address the question of how often to do endoscopies looking for GC in the family members. I would do one endoscopy with a careful look for any suspicious raised or depressed lesions. Antral and body biopsies as discussed above would give an overview of whether there is extensive IM. Thereafter I would do periodic endoscopy every 3 to 5 years.
