Future Directions

A number of genes responsible for hereditary CRC predisposition have been identified, and genetic testing has been implemented in a variety of settings to influence medical management. However, likely 10 to 30% of CRC is familial, caused by the interaction of lower penetrance genes with environmental factors. Investigation continues to identify such genes through family studies. Although the currently known syndromes are autosomal dominant, future discoveries may be recessive. Recently, mutations of the MYH1 gene, a base excision repair gene, appear to be associated with the autosomal recessive development of multiple colorectal adenomas, similar to the colonic manifestations of attenuated FAP and/or FAP. Commercial testing for mutations in this gene is currently unavailable.
