General Approach to Treatment Neuroendocrine Tumors

Many of these disorders are due to a neuroendocrine tumor ectopically secreting a hormone causing the diarrhea. Therefore, each patient has two different processes requiring treatment: treatment of the hormone excess state (in this article, the diarrhea) and treatment directed at the tumor per se. The latter is important because 40 to 90% of these neuroendocrine tumors are malignant in different series. Although complete tumor resection would accomplish both treatment aims, it is frequently not possible because of the presence of metastases at presentation resulting in unresectable disease. The clinical state owing to the ectopic release of hormone should be treated first because, in some cases (especially VIPomas, gastrinomas), it can be life threatening. Although general treatments for the diarrheal state are frequently used, definitive treatment of each of these disorders requires that the correct diagnosis is made using the appropriate laboratory methods (ie, radio-immunoassays, biochemical assays, and measurements of hormonal activity).
