Insulated Tip Diathermic Knife

The insulated-tip diathermic knife (IT-knife) has a ceramic ball at the top of a conventional diathermic needle to prevent damage to the muscularis propria. After a submucosal injection, a 2 mm incision is made with a diathermic knife to make a hole for the insertion of the ceramic ball. The incision is performed around the lesion. When the circumferential incision has been completed, the resection is performed with a snare polypectomy. The IT-knife technique is difficult and can be associated with bleeding and perforation (Ohkuwa et al, 2001).
