Approximately 5 to 10% of colorectal cancer (CRC) is hereditary in origin, with linkage to a specific gene mutation. Moreover, some hereditary syndromes have an 80 to 100% lifetime risk for CRC, whereas others carry a 10 to 20% chance. Colorectal tumors are largely preventable through regular endoscopic evaluation, and some at-risk individuals require annual screening. However, there are also known risks and costs associated with repeated endoscopy. Furthermore, depending upon the specific syndrome, screening may be recommended as early as the age of 10 to 12 years, and some require prophylactic colectomy. Therefore, the key is to differentiate those individuals who would benefit from increased and/or earlier surveillance from those who would not. With the discovery of hereditary CRC genes, genetic testing, with accompanying management recommendations, has become the standard of care in some syndromes.
