Neoadjuvant Therapy

Because many GC patients are significantly debilitated postoperatively, neoadjuvant therapy has gained attention as a possible approach for patients with advanced GC. One potential benefit is that patients are more likely to tolerate an aggressive chemoradiotherapy regimen preopera-tively. The in situ tumor can be used as an in vivo control to assess response to treatment, downstage the tumor, and to allow resection that was not possible before treatment. These benefits are balanced by the increased operative morbidity that is more likely to occur in a previously irradiated field and the potential for over treatment of patients who will not benefit from this regimen. Trials evaluating both neoadjuvant protocols and adjuvant approaches (intraperi-toneal, immunotherapy) are currently ongoing.
