Persistent Diarrhea

Treatment of persistent travelers' diarrhea depends on the identification of the etiologic agents. This, of course, requires stool evaluations for protozoa. The rule of thumb is still three stools collected over several days. In some instances a string capsule may be useful in identifying Giardia in the upper small intestine. With the exception of Entamoeba histolytica infection, serology is not useful in making these diagnoses. Metronidazole or tinidazole (Presutti Laboratories, Arlington Heights, Illinois) are the drugs of choice for both Giardia and E. histolytica. Trimethoprim-sulfamethoxazole is the drug for Cyclospora. The only drug that has been shown to be effective against Cryptosporidium is nitazoxanide, but experience with the drug has not yet been extensive.
