This syndrome is traditionally described in the critically ill patient population. It is often associated with severe trauma or burn injury, nonbiliary tract surgery, and prolonged fasting. It usually has a more fulminant course with an increased incidence of gangrene, empyema, and perforation. Affected patients present with fever, increasing abdominal pain, elevated liver function tests, and leuko-cytosis. These patients are often deemed too ill for chole-cystectomy due to their underlying disease processes. Therefore, cholecystostomy tube placement is frequently undertaken. The latter may be performed under either general anesthesia or with conscious sedation.
