The most common symptom is right upper quadrant pain and is often associated with anorexia, nausea, or vomiting. Fever is frequently present, but is not required for the diagnosis. On physical examination, right upper quadrant tenderness is common and may be severe with localized guarding or rebound. The gallbladder may be palpable in some patients, but a Murphy's sign (inspiratory arrest during palpation of the gallbladder) is almost always present. Mild jaundice is not uncommon, although bilirubin rarely exceeds 6 mg/dL. Laboratory examination frequently reveals leukocytosis of 12,000 to 15,000 cells/mm3, but a normal white blood cell count does not exclude the diagnosis of acute cholecystitis. Liver function tests (bilirubin, transaminases, and alkaline phosphatase) may be elevated or normal.
