Preventing AC of the Esophagus

There is only one form of therapy that is clinically believed to prevent the development of AC of the esophagus. It is an esophageal resection, once high grade dysplasia (HGD) has been documented. This is assuming that the patient has only HGD and, in fact, is not harboring an occult AC that has already spread to regional lymph nodes. In addition, there is emerging data that photodynamic therapy used to endoscopically ablate HGD can reduce the development of AC of the esophagus as described in the next chapter (see Chapter 15, "Ablative Therapy in Esophageal Disease"). In the face of cancer risk, we are left with surveillance endoscopy as a tool of early detection and potential intervention to improve survival. On the horizon is the possibility of chemoprevention as a strategy to decrease the development of AC.
