Malignancies involving the liver represent a difficult and controversial indication for transplantation. Liver transplantation plays an important role in the management of primary HCC and hepatoblastoma. However, that role is limited by the potential for recurrence. For transplantation to be considered, there should be little or no potential for effective nontransplant therapy and little potential for recurrence after transplantation. Meeting these criteria narrows the field of candidates and excludes patients with cholangiocarcinoma or hepatic sarcoma. Transplant therapy should not be considered in the treatment of metastatic disease to the liver.
