Long term biochemical cure is difficult or impossible to achieve in patients with MEN I-associated gastrinoma. Almost all patients with MEN I will have recurrent disease and 10-year biochemical cures rarely occur. In contrast, 50% and 35% of sporadic gastrinoma patients undergoing surgical resection may be disease free at 5- and 10-year follow-up, respectively. Up to 93% of patients with MEN I-gastrinoma are alive 15 years after diagnosis, provided appropriate medical therapy is maintained. This, combined with the difficulty in achieving biochemical cure in MEN I has led some to conclude that aggressive surgical management should be reserved for the sporadic form.
