The available sclerosants and their dosing regimens are summarized in Table 28-2. The benefit of injecting a sclerosant following epinephrine is controversial. Some studies have shown a benefit, especially in those with spurting hemorrhage (Lin et al, 1993), whereas others have not. Because of the risk of gastric necrosis and the lack of added benefit to epinephrine, we do not recommend adding sclerosants after epinephrine injection. Several studies have shown good results for endoscopic injection of ethanol, but possibly with a higher risk of local complications and perforation.
