Surgical intervention is necessary when colonoscopic decompression has failed on three attempts, there is a danger of impending cecal perforation, or when the patient has signs and symptoms or peritonitis and cecal perforation. Surgical intervention is associated with mortality of 20 to 40%. Right hemicolectomy, ileostomy and mucus fistula is performed for complicated pseudo-obstruction with perforation. In patients with ischemic bowel without perforation, a surgical consultation should be obtained. In patients without ischemia or perforation, percutaneous or laparo-scopic cecostomy has been used to decompress the colon.
