Colectomy has been performed in a handful of patients. In one patient, diarrheal symptoms were eliminated, and in another, diarrhea and extraintestinal manifestations of col-lagenous colitis were abated. However, one patient continued with diarrhea after colectomy for unclear reasons. Diverting ileostomy has regressed the clinicopathologic findings in this disorder. Jarnerot and colleagues (1995) reported nine women with collagenous colitis unresponsive to medical therapy (sulfasalazine, mepacrine, corticosteroids, mesalamines, metronidazole [Flagyl]). An ileostomy was performed in eight patients and a sigmoi-dostomy in one patient. Postoperatively, diarrhea ceased in all patients, with histologic resolution of the collagen layer. The ileostomy was reversed in three patients after a diversion period of 4 to 15 months, and clinical symptoms and the abnormal collagen layer recurred in all.
