Systemic Chemotherapy

The choice of systemic chemotherapeutic agents and their efficacy varies with respect to the tumor type. For advanced colorectal cancer, a variety of cytotoxic agents have been tested in patients with advanced disease in the liver and elsewhere. The most useful chemotherapeutic regimens are typically drug combinations including 5-fluorouracil, leu-covorin, and either irinotecan or oxaliplatin. Newer biologic agents, such as cetuximab and bevacizumab, are showing early promising results in advanced colorectal cancer as well. Recent studies with these newer regimens demonstrate response rates exceeding 50% with significant improvement in survival. Systemic therapy for metastatic neuroendocrine cancer is of limited benefit.
