TIPS and Surgical Shunts

TIPS and surgical shunts are more effective than pharmacologic or endoscopic therapy at preventing rebleeding, but are not recommended as first line therapy due to their side effects (see above). Additionally, there is no improvement in survival from TIPS or surgical shunts when compared with endoscopic therapy, and studies have found significantly higher mortality in patients undergoing TIPS on univariate, but not multivariate, analysis. However, in patients who fail treatment with ß-blockers and endoscopic variceal ligation, TIPS and surgical shunting (for Child's A cirrhotics) may be lifesaving.
