Acceptable methods to measure hemodialysis adequacy as recommended in the Dialysis Outcomes Quality Initiative (DOQI) Clinical Practice Guidelines. These guidelines may change as new information on the benefit of increasing the dialysis prescription becomes available. For the present, however, they should be considered the minimum targets.

'Recommended by the National Kidney Foundation Dialysis Outcomes Quality Initiative Clinical Practice Guidelines, which suggest a prescribed minimum Kt/V of 1.3 and a minimum urea reduction ratio of 70%. 'Ln is the natural logarithm; R is postdialysis blood urea nitrogen (BUN)/predialysis BUN; t is time in hours, Uf is ultrafiltration volume in liters; w is postdialysis weight in kilograms.
