Pyelonephritis in females. Essentially, this is an ascending infection caused by uropatho-gens. From the perineum the bacteria gain access to the bladder, ascending to the renal pelvocalyceal system and thence to the renal medulla, from which they spread toward the cortex. It has been shown that "pyelitis" cannot be considered a pathologic entity, as renal pelvis infection is invariably associated with nearby contamination of the renal medulla.
